The Failure bottle or the useless item is part of Leshy's deck-building card game that players have to partake in. Players can only use items during combat, and each item has a different use. But the Failure bottle doesn't seem to have any listed uses in the game; its page in the rulebook just says it can't be used.
The Boon of Magpie's Eye is a Boon given to the player during Leshy's Trials in Act I of Inscryption. This boon allows the player to, when drawing from their deck, chose any card from their deck to draw.
